We are so excited to meet you and your child! To ensure a smooth registration process, please complete the required forms before your appointment in the language you are most comfortable with. You can access them here:

✅ Your photo ID (e.g., driver’s license or passport)
✅ Your child’s original birth certificate or passport
✅ Two proofs of address (e.g., lease, mortgage statement, utility bills)
✅ Updated immunization records
✅ Completed registration forms
